Ordered By

Displays the name of the person who ordered the delivery.

Signature

Displays the name of the person who signed for the delivery.

Delivered By

Displays the name of the person who delivered the equipment to the customer job site.

Delivery Date

Displays the date that each delivery was made.

Code

Displays the code that prints on the invoice when there are delivery charges.

Amount

Displays the amount to charge for each delivery.

F3	Exit. Exit the page or program.
